Podiatry self-referral

The Podiatry team care for patients who have problems with their feet or lower limbs.

They can assess, diagnose and manage foot and leg problems to help reduce pain, prevent deterioration of chronic conditions and increase mobility.

They treat people who have a medical/podiatric problem that puts your foot at risk of deterioration.

Conditions include;

• painful corns and callus
• ingrowing toe nails
• foot ulcers

They do not treat verrucae (warts), fungal nails, normal nails or provide surgical shoes.

Simply download and complete the self-referral form and email your completed form directly to buc-tr.podiatry@nhs.net. Please do not send your completed forms to the Practice.
