Registering with the Practice

We are using an online service called Register with a GP surgery that makes it easy to register with this practice.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status or ID.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

When you register, it is helpful to have your NHS number. You can use the NHS website to find your NHS number.

Accessing someone else’s information on the NHS App

Some patients find it helpful for a second person to have access to their online GP record. This is often a family member, medical next of kin, a close friend or a carer whom they trust to act on their behalf.  We can only set this up for you if you are both registered with us.

To requests proxy access:

• Fill in our admin request form
• Or visit reception between 10 am and 6 pm and pick up a paper registration form

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website. The NHS website has information about using linked profiles to access services for someone else.

If you would like us to discuss your information with someone else

You can nominate a family member, medical next of kin, a close friend or a carer whom you trust to act on your behalf.

To request this please:

• Complete a Letter of consent
• Or visit reception between 10 am to 6 pm and pick up a paper consent form

Named Accountable GP

From the 1st April 2015 the NHS requires that every patient is allocated a named accountable GP. All registered patients have been allocated a named GP and any newly registered patients will be allocated a named GP within 21 days of registering. This is for administrative purposes only and you retain the right to see any of our GPs. You will still be able to book an appointment with the GP of your choice.
