Before completing a registration form please check you are in our catchment area , or email our team to confirm your eligibility.
We are using an online service called Register with a GP surgery that makes it easy to register with this practice.
Just fill in the quick online form to start the process. The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.
We are a Safe Surgery for everyone in our practice area. We might ask for OD or proof of address. But if you don’t have any and you live in our practice area, you can still register with us. We won’t ask for immigration documents.
When you register, it is helpful to have your NHS number. You can use the NHS website to find your NHS number.
In response to the Infected Blood Inquiry, NHS England is working to identify individuals who may have received infected blood through a transfusion before 1996. Evidence shows that, following the introduction of universal blood screening in September 1991, the risk of contracting a bloodborne infection from transfused blood has become extremely low.
If you have indicated on your patient registration form that you received a blood transfusion prior to 1996, and you have not previously been tested for hepatitis C — or are unsure whether you have been tested — we recommend that you undergo a blood-borne virus screening. This includes testing for hepatitis C

Some patients find it helpful for a second person to have access to their online GP record. This is often a family member, medical next of kin, a close friend or a carer whom they trust to act on their behalf.  We can only set this up for you if you are both registered with us.

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website. The NHS website has information about using linked profiles to access services for someone else.

From the 1st April 2015 the NHS requires that every patient is allocated a named accountable GP. All registered patients have been allocated a named GP and any newly registered patients will be allocated a named GP within 21 days of registering. This is for administrative purposes only and you retain the right to see any of our GPs. You will still be able to book an appointment with the GP of your choice.
