Patient Support Service

This NHS service is open to all patients over the age of 18 years old, who are registered with the PCN surgeries.

The North Bucks Patient Support Service team is comprised of Social Prescribers, Care Coordinators and Health & Wellbeing Coaches. We offer home visits where we can discuss issues that are important to you in order to improve your health and wellbeing. We can then signpost you to appropriate services or community groups.

This may be regarding:

· Living well and more independently

· Being more socially connected

· Improving your physical and mental health

· Being more active

· Managing your finances and living conditions

· Accessing education and improving your skills

We work alongside a wide range of health professionals and local organisations. We also support our Telephone Befriending Service. This group of volunteers enable people, who cannot easily access social groups, to speak to someone each week for a friendly chat.
