Oving Road will be closed for 3 Days. ALTERNATIVE ROUTE FOR AFFECTED USERS Marston Road, Winslow Road, Horn Street, Sheep Street, Aylesbury Road, Hurdlesgrove Hill, Buckingham Road, Oving Road and vice versa. Please allow more time to get to your appointment as you will need to go the long way round.

Patient Support Service

The Patient Support Service is provided free of charge by four GP Practices in North Buckinghamshire.

Social Prescribers are part of the Patient Support Services and provide a free and confidential service that supports people with a range of social, emotional or practical needs with a focus on improving health and wellbeing.

The social prescribers work with many services including;

  • Doctors & other clinical professionals
  • Adult Social Care
  • Mental Health Services
  • Citizens Advice Bureau
  • Voluntary and charitable organisation
  • Local Councils and Housing Associations

A member of their team will either telephone you or visit you at home and together you’ll complete a holistic assessment. This will enable them to find the most and direct services that can help.

Some of the areas they can help with are

  • Housing, benefits, financial support and advice
  • Access to Social Care and care services
  • Employment training, education and learning opportunities
  • Accessing support for your mental health and wellbeing
  • Volunteering opportunities
  • Support for carers

They can then

  • Make a referral or signpost you to appropriate organisations
  • Support you in accessing social groups, activities and support groups
  • Remain a point of contact for your future concerns
